The NSW Public Health (Amendment) Act 1992 requires
parents to provide the school with an Immunisation
Certificate or Immunisation History Statement. The
Immunisation History Statement can be obtained by
contacting the Australian
Childhood Immunisation Register on 1800 653.
You can get an Immunisation Certificate from:

Your doctor
(General Practitioner)

Local Council
(immunisation clinic)
-
Medical Officer
of your nearest Public Health Unit
Immunisation is not compulsory. If you chose not to
immunise your child you must still provide the school
with an Immunisation Certificate stating that the child
is NOT immunised. In the event of a vaccine preventable
disease occurring in the school, unimmunised children
will be required to remain at home for the duration of
the outbreak, as directed by the local Public Health
Unit.

Chicken Pox |
Exclude for 7
days after the spots appear.
Contacts are not excluded. |

Rubella
(German Measles) |
Exclude for 7
days from first appearance of rash.
Contacts are not excluded. |
|
Measles |
Exclude for 5
days from appearance of rash or until medical
certificate of recovery is produced. Contacts
are not excluded. |

Mumps |
Exclude for 10
days from onset of swelling.
Contacts are not excluded. |
|
Hepatitis |
Re-admit on
medical certificate. Contacts are not excluded. |
|
Ringworm |
Excluded until
all evidence of infection has disappeared or a
medical certificate is produced stating that the
lesions are inactive. |

Impetigo
(School Sores) |
Exclude only if
on exposed skin surfaces such as scalp, face,
hands or legs. |
|
Pediculosis (Head Lice) |
Treat using lice
shampoo and wash items that their hair has come
in contact with. |
